How common is this? What can be done to prevent it other than not giving large meals?
I think bloat varies from dog to dog. In reality nothing can be done to prevent it. I've heard everything from small meals, no water after eating, raised food bowls, no exercise before or after eating...the list goes on and on. The best thing it to know the symptoms and know to get help asap.

Gastropexy (tacking the stomach to the side of the abdomen) can keep a dog from bloating with torsion, but they can still bloat and die from it. Avoiding torsion buys one precious time in getting them to the vet.

Do elevated feeders reduce or increase the risk of bloat? I have read things for both sides. Do most of you use elevated feeders and recommend them?
